This blog describes the writup for problem solving in PI:

In an effort to solve the issue , we sometimes do not consider the actual cause of the issue. This leads to disrupted solving of the issue and the next time this occurs we do not recollect any issue again.

Hence to avoid such issues we HAVE to follow a write-up for the following. This write-up is to be used when we interact with any user and to make sure we have a clear understanding of what is happening.

For Future PI issues for which a write up is required and this Write up's will be helpful for others to analyze when the issue reoccurs. Always prepare the Write UP's and pass it on to our team, please discuss on the write up's in the coming days. This information will further helps us in doing better PI Monitoring.
