In customBeforeShowScreen(), he determines which HTML element is the form of the screen being shown, and adds an image to the top of the element with: topOfFormElem.innerHTML. He also does some basic customization to the jQueryMobile.css file to change the background gradient color (clarification: SUP 2.0 developers can add custom .css files to their app projects).
