One possible soultion could be [QuickPoll|http://help.sap.com/saphelp_nw04/helpdata/EN/1d/c5e6591409cd4ca5c939bceb912417/frameset.htm] - but I don't think it could be used for extended survey.
Another option is to create XML Form builder template with questions and answers, then collect the XMLs and evaluate the answers from them.

As an alternative to all of the above mentioned by Meena Rane you may consider about the usage of Visual Composer.
