Never happened to me nor seen it reported.
Never seen that. Did we ask the user to do it again to see if the problem reoccurs?
This person created a document, I think they meant to create a discussion. Not sure if it is a mistake or if it applies to this but thought it couldn't hurt to mention:

Not yet. Since the order of the items in the actions menu has been changed recently, maybe he clicked rather blindly in the accustomed area?
