Does MDM be upgarded on a ECC server ? or it has to be installed as a separate server to get linked to ECC?

mdm is not connecting to any system, you have to setup the connection via some middleware like pi
It's a completely different package and needs to installed separately. Detailed documents are available on help.sap.com

mdm will generate the xml(customer, supplier...) file, which you have to feed to the ecc by some means.
mdm has nothing to do with ecc
it is another product acquired by sap
