unable to find Report Safety Observation link to creation process

Hi  Everyone ,

we installed EHSM  3.0

In the standard system of portal

assigned all roles to the user

i can find Link to Report  Incident and Near Miss

But

i could not find tab or link to Report  Safety observation

Please find the attached screen shot  can you please guide me how to create safety observation

can any one help me out in this regard
