IT0021(Family members/Dependents) showing two spouse records

Hi All,

Actually I have an issue with IT0021.

The  sub type spouse records showing 2 records.

case 1: same spouse record showing 2 records.

case 2: 1 record with actual spouse and 1 record tagging with some other person.

In DEV System It's not happening..we are encountering in Production..

Provide some inputs to troubleshoot.
