Could you please cross check Access Request Approval form has been changed directly in quality ?
I mean Is that approve button made in visible via SE80 ?
As you said Dev is ok, then please collect it Access Request Approval form in transport from dev to qa.

are you not getting this submit button at a particular stage or in all approval stages. can you tell what are the approval stages and which stage you are getting this issue.
