Greetings Sonal,
To try and answer your questions in a nutshell:
1) A Work Clearance Application is a document that is created by a "Work Requestor" (e.g. maintenance planner) in a Plant to make a request to the Operations or Operational Security department or responsible to make necessary security preparations and allow a maintenance team to go to the plant floor to perform necessary work. In some jurisdictions, this may be a legal document, or an internal one.
A WCA exists both in the Standard and Enhanced model of WCM. It is created on top of a Work Order and used to track the necessary Approvals, safety measures and link to WCD documents, which track the necessary activities to secure and lockout/tagout the installation. The WCA can be assigned if the Planning Plant has the relevant WCM configuration and the Work Order has a relevant Permit (usually called "WCM")
2) Valuation means that for the Work Orders you are to decide whether the Order is relevant to a Work Clearance Management process or not. If Valuation is active, this distinction has to be made for each Work Order. If it is inactive, then the distinction is decided by the Permit mentioned. (You may have WCM for some works but not others in the same plant, for example, you will need to shut down and secure an assembly to inspect it, but you do not need lockout/tagout to paint a safety railing around the assembly, or a room in the administration building.)

3 & 4) A Work Clearance Document is used to specify and track which technical objects (Functional Locations, Equipments and others) need to be prepared and in what manner to allow the maintenance team to come in to perform the works. This is the "lockout/tagout process" and it may involve e.g. switches to disconnect, valves to open or close, blockades to apply etc. This document can be linked to the WCAs and only after the WCD is processed, the work may be safely performed. After work is performed, the WCD is finalized and closed.
