we have the requirement that an opportunity should only have one item.
Is this possible to realize with Customizing? e.g special item type?

You may try badi CRM_ORDERADM_I_BADI
Write your own logic in CRM_ORDERADM_I_CHECK or merge to raise error and abort inserting new item.
